Admission

Parkside PAR welcomes all those in need of care without regard to race, age, disability, color, national origin, religion, marital status or gender. We will make no determination regarding admissions or discharges based upon these factors.

The transition from hospital to a rehabilitation center can be a stressful process. There are so many decisions to make and information to pull together. If you have any questions or need help taking the next step we are just a phone call away.

Though the list below is not exhaustive, it may prove helpful as you begin gathering the information required for medicare or other insurance providers.

Verification of Citizenship

  • Birth Certificate
  • Driver’s License
  • Voter Registration Card

Verification of Social Security Numbers

  • Medicare Card
  • Social Security Card

Verification of Resources

  • Checking Account(s) – last 3 months bank statements prior to entering the nursing home
  • Savings Account(s) – last 3 months bank statements prior to entering the nursing home
  • CDs

Verification of Income

  • Social Security Award Letter
  • Retirement Award Letter
  • Annuities
  • VA
  • Railroad Retirement
  • Other Retirement

Deeds to Property

  • If applicant/ spouse inherited or transferred any property in the past 24 months or if the applicant/spouse have a lifetime interest in home place property/any other property.

Information on Life Insurance

  • A copy of the policy face amount and policy amount not to exceed $10,000 – anything over this amount the state of Georgia must be placed as the beneficiary

Verification of Burial Plot or Burial Funds

  • A copy of the burial contract or burial account

Automobiles

  • A copy of current tag receipt

Insurance Information

We will not determine admissions or discharges based upon these factors. It is our goal to make the transition process as easy as possible. We’ve provided a few links below along with a list of the insurance companies we are currently able to work with.

Discharge

Embracing the Journey means that we are not only committed to caring for you while you are with us;
we offer support even after you leave Parkside.

  • Our discharge team can coordinate services for return home or back to the community. This may include scheduling follow up appointments with the primary physician.
  • We are able to assist with durable medical equipment that may be needed post-discharge.
  • Based on the individual care needs after discharge they may have home health or home care services arranged.
  • Through pharmacy provider; we can assist with the post discharge pharmacy overview ( the pharmacist will follow up after discharge to assist with any medication concerns to help prevent re-hospitalizations).
